Job Description

  • Provide case management services for participants in the Women and Children Supportive Housing Program.
  • Conduct intake assessments
  • Connect with clients biweekly for 6 months to a year
  • Document and submit billing for assessments and services
  • Motivate clients to stay engaged in outpatient substance abuse treatment
  • Enroll uninsured clients in health insurance programs
  • Refer clients to programs in Baltimore City, County and Anne Arundel County
  • Extensive experience in case management
  • Bachelors degree in Human Services field
